Statement of Credit Agreement: Pending application approval, the undersigned agrees to the
following conditions of credit with AuralTech CD Refinishing: The account of the above named
firm, corporation, partnership or other entity will be paid in full within (40) forty days of
the invoice date. Statements may or may not be issued. The undersigned further agrees that if
the debts incurred by the business entity are not kept current, that, solely at our discretion,
they may be held immediately and individually liable for all debts plus all interest and
carrying charges, attorney fees, court and collection costs. Venue of any action shall lie in
King County, Washington. Past due accounts will be assessed a finance charge of 1½% per month
(18% per annum), or the highest legal rate applicable, not in excess of 18% per annum. In the
event of a dispute between AuralTech CD Refinishing and the undersigned regarding any terms
or conditions of this agreement, or resulting from the purchase of any service or goods, said
dispute, claim or suit shall be resolved by binding arbitration in King County, Washington.
AuralTech CD Refinishing does not extend credit on a consumer, non-business basis.
The undersigned hereby warrants that all purchases made from AuralTech CD Refinishing
will be used solely for commercial purposes in the furtherance of a business. The below
signed certifies that the above statement has been carefully read and agreed to, and all
representations made are correct.
